“How to Get An Evaluation for Your Child Through School” (Ages 3-21 years of age)



If you feel your child has a disability or has been diagnosed with a disability you may want to request an evaluation through school for special education services. Check out our Information Sheet on “How to Get An Evaluation for Your Child Through School” (Ages 3-21 years of age). A sample letter requesting your child be evaluated can be found in NICHCY's Communicating with Your Child’s School Through Letter Writing on page 12.

  1. My child is 10. He was born @ 37 wks and 5 days...3lb 11oz. I am having trouble getting his school to do anything. He has had a Tier 3 Test and 2 IQ test (72 then 75) He is considered Mildly mentally disable. He is below avg and well below avg. Below his peers and well below his peers on everything. He has ADHD and some other problems. They say he is in the "Grey Area" and apparently those kids don't matter. What can I do?

  2. ADHD falls under the Other Health Impaired (http://nichcy.org/disability/specific/ohi) category for IEP eligibility (Categories of Disability under IDEA http://nichcy.org/disability/categories). Follow steps above in article and send letter requesting evaluation under OHI category, a sample letter can be found Communicating with the School Through Letter writing (http://nichcy.org/wp-content/uploads/docs/pa9.pdf) page 12. The school will give you a form your doctor will need to complete for ADHD.
    In response to Tier 3, RTI please see this OSEP 11-07 Response to Intervention (RTI) Memo, Jan. 21, 2011 (http://www2.ed.gov/policy/speced/guid/idea/memosdcltrs/osep11-07rtimemo.pdf) guidance document issued by US Department of Education as guidance to the states explaining Response to Interventions (RTI) cannot be used to delay requested evaluation under IDEA.
    Kentucky Department of Education (KDE) has developed a guidance document, A Guide to the Kentucky System of Interventions, (http://education.ky.gov/educational/int/ksi/Documents/KSIRtIGuidanceDocument.pdf) to provide resources in planning and decision making as districts and schools develop a system of interventions to meet the needs of all students.
